Teva Clozapine Prescribing Reminders

Patients must be registered with the Teva Clozapine Patient Registry prior to dispensing Teva Clozapine. Call 800-507-8334 for assistance.

A current and acceptable WBC count and ANC value is required prior to dispensing clozapine and for 4 weeks after discontinuation of treatment.

A current and acceptable lab count is defined as WBC greater than or equal to 3,500/mm3 and ANC greater than or equal to 2,000/mm3 and no older than 7 days from the dispense date.

Submit WBC/ANC lab reports to the Teva Clozapine Patient Registry within 7 days of the blood test.

A one, two or four week supply can be dispensed depending on the patients monitoring frequency.

Patients are monitored weekly for the first six months. If there are no low counts the patient can be monitored every two weeks for an additional six months. Afterwards, the patient may qualify for every 4 week monitoring.

Every 4 week monitoring requires:

  • Physician Authorization (For a copy of the Monthly Monitoring Request Form, Click here).
  • Clozapine treatment for at least 1 year,
  • Every 2 week monitoring for at least the past 6 months,
  • Acceptable WBC and ANC values over the past year

  • Transitions to reduce frequency of monitoring to every 4 weeks only permitted if all labs within the past year are acceptable (WBC counts >= 3,500/mm3 and ANC values >= 2,000/mm3).
