The Teva Clozapine Patient Registry is committed to safeguarding your data. Please read the following information to understand how we maintain the security of this website and the confidentiality of patient information.
This website has a public access component and a secure access component. The public access component provides general information about the Teva Clozapine Patient Registry to any web browser. The secure access component is available only to registered users and requires a User ID and Password. User ID and Passwords are activated by Teva Clozapine Patient Registry staff only after manual verification that a user request is valid. Once access is granted, the user will have visibility to only their set of patients.
Security of data transmissions is maintained by using international 128-bit data encryption standards. All data transmissions are encrypted and require a browser that supports 128-bit Secure Socket Layer Protocols. In addition, a multi-level security system is used to prevent unauthorized access to patient data.
With regard to the Patient Privacy Rules under the Health Insurance Portability and Accountability Act of 1996 ("HIPAA") please note the following. The Teva Pharmaceuticals Clozapine Patient Registry is a systematic active postmarketing surveillance program mandated by the United States Food and Drug Administration ("FDA"). Under this mandate, Teva is required to collect patient lab data, patient identification information and investigate adverse events in connection with its clozapine product. The patient privacy regulations under HIPAA permit disclosure by health care professionals of "protected health information" to fulfill postmarketing surveillance requirements as mandated by the FDA without patient consent. See, 45 CFR Section 164.512 (b)(iii); Uses and disclosures for which consent, an authorization, or opportunity to agree or object is not required.
